*If there is more than one doctor in the practice involved in the Physics Forceps evaluation, we ask that each doctor register individually.
Terms of Participation:
You have been selected to participate in a pre-market field evaluation for Physics Forceps. The field evaluation is part of an ongoing product development effort by GoldenMisch, Inc. and Physics Forceps, LLC. Due to the pre-market nature of this, all information learned and/or shared regarding Physics Forceps between you and GoldenMisch, Inc. and its representatives is sensitive and considered CONFIDENTIAL. By agreeing to participate in this evaluation, you also agree that you will not disclose confidential information or show or discuss Physics Forceps with any individuals who are not employed by the practice, and/or individuals who are not under the immediate care of the practice as a patient.
Your participation is strictly voluntary. Evaluators expect to receive no remuneration and they are under no obligation to make any purchase. GoldenMisch, Inc. is required by FDA regulation to collect field data as a means to validate the product design and efficacy. Therefore, we require that Evaluators submit written documentation to GoldenMisch, Inc. for all extraction cases where Physics Forceps and the One Minute Extraction Technique are used. A minimum of 15 documented extraction cases are requested from each evaluator. If you wish to continue using the evaluation product for the duration of the evaluation period, scheduled to close May 31, 2007, you are welcome to do so, but we ask that you also continue to submit documentation for occasional extraction cases.
Finally, you assert that you have no financial interest in Physics Forceps, LLC or GoldenMisch, Inc.
